Regardless of whether the assessment is mandatory or voluntary, effort will be made in scheduling to minimize the inconvenience to you, your patients and your staff. You or your office manager will be contacted to discuss options for mutually agreeable dates and times.  For dental practices volunteering for an assessment, the day will be of mutual convenience and availability.

If in the unusual circumstances a date cannot be identified for a mandated assessment by mutual agreement, the Director of Facility Assessments will identify a limited number of options to you or your office manager and you may select the most convenient day from those options.
